Electronic Visa Application Form(EVAF)

Electronic Nonimmigrant Visa Application Mandatory Starting November 01, 2006

All nonimmigrant visa applicants will be required to complete an ELECTRONIC VISA APPLICATION FORM (EVAF) DS-156 to apply for nonimmigrant visas.  The electronic DS-156 visa application forms can be downloaded from this link.  The EVAF is also available in several languages in addition to English.

Please note that handwritten or typed application (DS-156) will no longer be accepted.  The Consular Section may only waive this mandatory requirement in emergency or exceptional cases.  Such waiver decisions would be made on a case-by-case basis.
